Rates are based on package/service purchased. At this time I am in network with BCBS but I do not accept insurance. However, I can provide paperwork for you to submit to your insurance so that they may cover part or all of the cost. Please keep in mind that Shelby Burns is not an insurance provider, and it is the clients’ responsibility to determine if their health insurance will cover the nutrition and wellness services. Cancellation Policy I ask that you provide 48 hours notice if you aren’t able to attend a previously scheduled appointment. If you are unable to provide 48 hours notice, a cancellation fee in the amount of half of the session will be applied.
